This notice describes how medical and dental information about you may be used and disclosed by Nhat Khanh Dentistry Inc., and how you can get access to this information. Please review it carefully.
Nhat Khanh Dentistry Inc. is the dental practice responsible for patient care and health records. KinDentists LLC may provide website, marketing, or management services to the practice and may handle information only as allowed by applicable agreements and law.
This notice applies to protected health information maintained by the dental practice, including information collected through patient forms, appointments, treatment, billing, insurance, and care coordination.
We may use or disclose your information to provide dental care, coordinate treatment with other providers, submit insurance claims, collect payment, run the practice, evaluate quality, train staff, and conduct other health care operations allowed by law.
Examples include sharing information with specialists, laboratories, pharmacies, insurance plans, billing services, technology providers, and other service providers who support the practice.
